*If gum disease is present, we may recommend a deep cleaning followed by a periodontal maintenance program.

AUTOMATIC RENEWAL UNLESS PATIENT OPTS-OUT
I understand that this authorization will remain in effect until I cancel it in writing, and I agree to notify Beach Dental in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. An Auto Renewal charge will be sent via email one time per year on the policy renewal date unless a credit card is kept on file with our office. If a credit card is on file, I acknowledge that the origination of credit card transaction to my account must comply with the provision of U.S. law. I certify that I am an authorized user of this Credit Card and will not dispute these scheduled transactions, as long as the transactions correspond to the terms indicated.
The Beach Dental Plan Covers: 2 Professional cleanings (1x per 6 months) (in absence of gum disease), 2 periodic exams, and
x-rays per one calendar year from the policy start date.
